Services and Supports in Brief
All of Hope’s services and supports are individualized. These are just examples of those commonly requested. Given the opportunity, Hope can usually meet the needs of most people requesting services.
Family Support—assists families with accessing community supports, acquiring specialized equipment and home modifications to prevent crisis and/or out-of-home placement.
Supported Living—individual’s choice of community living with assistance in activities of daily living.
Supported Employment—assistance with skill building in independent placements and community based employment opportunities.
Day Habilitation— assistance with skill development that enables a person to participate fully in their community.
Care Coordination—developing person-centered plans for support.
Foster Care/Family Habilitation—supports children and adults in loving family settings.
Shared Care—supports children in their own home with time spent in another family’s home.
In-Home Supports—provides the needed support to a family to keep their child at home.
Home Ownership—assistance with fulfilling people’s dreams of owning their own home.
Community Health and Wellness—supports individuals in choosing their own health professionals. Provides wellness supports to individuals who experience disabilities, families and employees.
Rural and Urban Subsistence Activities—assists people to engage in activities that are traditional to their family or culture.
Supported Parenting/Relationships—supporting families (couples with and/or without children & single parents) who experience disabilities to learn and complete activities of daily living, with an intense focus on relationships, familial roles, and parenting skills.
Mental Health—provides an array of mental health services for children and adults, including those with dual diagnosis.
Behavioral Health—assists individuals with managing their challenging behaviors so they may access their community and live independently as possible.
Recreational Opportunities— supports individuals in participating in inclusive structured recreation programs.
Spiritual Wellness— provides facilitation in connecting with faith communities, renewing the spirit, coping with illness and grief, along with other spiritual needs.
Self-Advocacy/Self-Determination—assists people in actively advocating for one’s self through education, support and mentoring.
